A patient is not always told when a student is performing a procedure for the first time. Withholding this information is a form of deception. It is justified on paternalistic grounds (it is in the patient's interest not to know), or on public policy grounds (given the choice, patients would refuse, thus compromising the training of future physicians). Using the spinal tap procedure (lumbar puncture) as a paradigm, 173 patients were surveyed to determine how they felt about first time procedures (...) by medical students, interns, and residents. The patients indicated that they would be willing to be the subject for a student's (52%), intern's (62%), or resident's (66%) first spinal tap. This paper reassesses the ethics of consent for first time procedures based on responses to this survey. (shrink)

Charles Avison's Essay on Musical Expression, first published in 1752, is a major contribution to the debate on musical aesthetics which developed in the course of the 18th century. Considered by Charles Burney as the first essay devoted to 'musical criticism' proper, it established the primary importance of 'expression' and reconsidered the relative importance of harmony and melody. Immediately after its publication it was followed by William Hayes's Remarks (1753), to which Avison himself retorted in his Reply. (...) Taken together these three texts offer a fascinating insight into the debate that raged in the 18th century between the promoters of the so-called 'ancient music' (such as Hayes) and the more 'modern' musicians. Beyond matters of taste, what was at stake in Avison's theoretical contribution was the assertion that the individual's response to music ultimately mattered more than the dry rules established by professional musicians. Avison also wrote several prefaces to the published editions of his own musical compositions. This volume reprints these prefaces and advertisements together with his Essay to provide an interesting view of eighteenth-century conceptions of composition and performance, and a complete survey of Avison's theory of music. The last two decades have seen two significant trends emerging within the philosophy of science: the rapid development and focus on the philosophy of the specialised sciences, and a resurgence of Aristotelian metaphysics, much of which is concerned with the possibility of emergence, as well as the ontological status and indispensability of dispositions and powers in science. Despite these recent trends, few Aristotelian metaphysicians have engaged directly with the philosophy of the specialised sciences. Additionally, the relationship between fundamental Aristotelian concepts—such (...) as "hylomorphism", "substance", and "faculties"—and contemporary science has yet to receive a critical and systematic treatment. _Neo-Aristotelian Perspectives on Contemporary Science _aims to fill this gap in the literature by bringing together essays on the relationship between Aristotelianism and science that cut across interdisciplinary boundaries. Hegel and Speculative Realism has two main objectives. Firstly, to assess the speculative realist formulations of the real regarding the ‘withdrawn’ object, radical contingency, the absolute register of extinction, and the current interest in ‘powers philosophy’, with special attention to their possible relation to the absolute scope of Hegelian philosophy. Secondly, to invite the reader to reconsider Hegel in a new way; uncovering rare insights into his thoughts on astronomy, actuality, the concrete and non-being. Johns’ inclination is to not mistake (...) the necessary path to the absolute as the only path. Johns argues that Hegel describes the unique trajectory of the dialectical relationship between Nature and Idea as a Spirit oriented by both logical and physical (spatio-temporal) dimensions. Johns reads this as a theory of singularity and makes the bold claim that there may be other paths not taken by the Hegelian spatio-temporal path synonymous with the dialectic; synthesis, sublation and unfolding. In-fact, speculative philosophy should not be satisfied to study only “what exists” but also what “could exist” or what it means to “inexist” and should entertain multiple modes of potential becoming between Hegel’s initial triad of logical categories; Being, Non-Being and Becoming. This study identifies and explores evolving concepts of trust and privacy in the context of user-generated health data. We define “user-generated health data” as data captured through devices or software and used outside of traditional clinical settings for tracking personal health data. The investigators conducted qualitative research through semistructured interviews with researchers, health technology start-up companies, and members of the general public to inquire why and how they interact with and understand the value of user-generated health data. We found significant (...) results concerning new attitudes toward trust, privacy, and sharing of health data outside of clinical settings that conflict with regulations governing health data within clinical settings. Members of the general public expressed little concern about sharing health data with the companies that sold the devices or apps they used, and indicated that they rarely read the “terms and conditions” detailing how their data may be exploited by the company or third-party affiliates before consenting to them. In contrast, interviews with researchers revealed significant resistance among potential research participants to sharing their user-generated health data for purposes of scientific study. The widespread rhetoric of personalization and social sharing in “user-generated culture” appears to facilitate an understanding of user-generated health data that deemphasizes the risk of exploitation in favor of loosely defined benefits to individual and social well-being. We recommend clarification and greater transparency of regulations governing data sharing related to health. (shrink)

A rich literature in public health has demonstrated that health is strongly influenced by a host of environmental factors that can vary according to social, economic, geographic, cultural or physical contexts. Bioethicists should, we argue, recognize this and – where appropriate – work to integrate environmental concerns into their field of study and their ethical deliberations. In this article, we present an argument grounded in scientific research at the molecular level that will be familiar to – and so hopefully more (...) persuasive for – the biomedically-inclined in the bioethics community. Specifically, we argue that the relatively new field of molecular epigenetics provides novel information that should serve as additional justification for expanding the scope of bioethics to include environmental and public health concerns. We begin by presenting two distinct visions of bioethics: the individualistic and rights-oriented and the communitarian and responsibility-oriented. We follow with a description of biochemical characteristics distinguishing epigenetics from genetics, in order to emphasize the very close relationship that exists between the environment and gene expression. This then leads to a discussion of the importance of the environment in determining individual and population health, which, we argue, should shift bioethics towards a Potterian view that promotes a communitarian-based sense of responsibility for the environment, in order to fully account for justice considerations and improve public health. By pointing to deep philosophical confusions endemic to cognitive science, Wittgenstein might seem an enemy of computational approaches. We agree that while Wittgenstein would reject the classicist’s symbols and rules approach, his observations align well with connectionist or neural network approaches. While many connectionisms that dominated the later twentieth century could fall prey to criticisms of biological, pedagogical, and linguistic implausibility, current connectionist approaches can resolve those problems in a Wittgenstein-friendly manner. We present the basics of a Vector Symbolic Architecture (...) formalism, inspired by Smolensky, and indicate how high-dimensional vectors can operate in a context-sensitive and object-independent manner in biologically plausible time scales, reflecting Wittgenstein’s notions of language-games and family resemblance; we show how “soft” symbols for such a formalism can be formed with plausible learning cycles using Sparse Distributed Memory, resolving disputes surrounding Wittgenstein’s private language argument; and show how connectionist networks can extrapolate meaningful patterns to solve problems, providing “ways to go on” without explicit rules, which indicates linguistic plausibility. Connectionism thus provides a systematicity and productivity that is more than a mere implementation of a classical approach, and provides Wittgenstein-friendly and Wittgenstein-illuminating models of mind and language for cognitive science. Charles W. Morris' doctoral thesis Symbolism and Reality, written in 1925 at Chicago under George H. Mead, has never before been published. It sets out to prove that thought and mind are not entities, nor even processes involving a psychical substance distinguishable from the rest of reality, but are explicable as the functioning of parts of the experience as symbols to an organism of other parts of experience. Being then the symbolic portion of experience, the psychical or mental can (...) neither be sharply opposed to the rest of experience nor identical with the whole of experience. William Desmond: It is a pleasure to welcome Professor Charles Griswold today. I thank him for his willingness to present us with an overview of his new book Adam Smith and the Virtues of Enlightenment , and to participate in a discussion. Professor Griswold is professor of philosophy at Boston University, where he is also the chair of the philosophy department. His new work on Adam Smith might seem like something of a departure from the concerns of many (...) of his prior publications. In particular I mean his writings on Plato and Platonic themes generally. I refer especially to his book Self-Knowledge in Plato's Phaedrus, first published by Yale University Press, and recently reprinted by Pennsylvania State University Press.This book is a close reading and interpretation of the Phaedrus, and was awarded the Matchette prize by the American Philosophical Association in 1987. Needless to say, Professor Griswold has written extensively on classical philosophy. This, however, cannot be separated from a concern with pressing problems of more contemporary currency, especially the role of philosophy in society, and with respect to ethical and political considerations. Hence his concern with the moderns, by contrast with the ancients, implies no slighting of the former, though the question persists as to what both have to say to us today. So it is not surprising to find him engaged with a very influential modern, Adam Smith: an influential, but also complex modern, in that themes from ancient thought receive their own distinctive configuration in Smith's thought.Enlightenment is often marked by a certain turn from the past, oriented to a putatively better future, via a reformed or revolutionized present. But the contrast with the past is sometimes less stark. This one might guess perhaps from the subtitle to the book, emphasizing the virtues of enlightenment. While Smith now is often remembered first as an economist, Professor Griswold's interest is directed to his work as a philosopher, especially his moral and political thought. Many of the themes that Adam Smith explored, and to which Griswold draws our attention, are still very live issues: the virtues, ethical reasoning, sympathy, moral education, the importance of ordinary life and the role of philosophical theory, to name but a few issues.Let me then welcome Charles again, and ask him to first offer us an account of his new work, its purposes and its claims. The myf5 and myoD genes are implicated in the specification of vertebrate skeletal muscle. These genes have been thought to be functionally redundant because neonatal mice bearing homozygous null mutations in either gene show grossly normal muscle development. By analyzing the early embryonic development of the mutants, Michael Rudnicki and coworkers show that trunk muscle development is retarded in embryos bearing myf5 null mutations, while early limb and branchial arch muscle development is retarded by myoD null mutations.1 These results indicate (...) that the myoD and myf5 genes are not redundant but that each controls the early specification of distinct muscle cell lineages. This book deals with the possibility of an ontological and epistemological account of the psychological category 'neurosis'. Intertwining thoughts from German idealism, Continental philosophy and psychology, the book shows how neurosis precedes and exists independently from human experience and lays the foundations for a non-essentialist, non-rational theory of neurosis; in cognition, in perception, in linguistics and in theories of object-relations and vitalism. The personal essays collected in this volume examine such issues as assimilation, the philosophy of neurosis, aneurysmal philosophy, and (...) the connection between Hegel and Neurosis, among others. The volume establishes the connection between a now redundant psycho-analytic term and an extremely progressive discipline of Continental philosophy and Speculative realism. (shrink)

Current theories suggest that the brain is the sole source of mental illness. However, affective disorders, and major depressive disorder (MDD) in particular, may be better conceptualized as brain-body disorders that involve peripheral systems as well. This perspective emphasizes the embodied, multifaceted physiology of well-being, and suggests that afferent signals from the body may contribute to cognitive and emotional states. In this review, we focus on evidence from preclinical and clinical studies suggesting that afferent thermosensory signals contribute to well-being and (...) depression. Although thermoregulatory systems have traditionally been conceptualized as serving primarily homeostatic functions, increasing evidence suggests neural pathways responsible for regulating body temperature may be linked more closely with emotional states than previously recognized, an affective warmth hypothesis. Human studies indicate that increasing physical warmth activates brain circuits associated with cognitive and affective functions, promotes interpersonal warmth and prosocial behavior, and has antidepressant effects. Consistent with these effects, preclinical studies in rodents demonstrate that physical warmth activates brain serotonergic neurons implicated in antidepressant-like effects. Together, these studies suggest that (1) thermosensory pathways interact with brain systems that control affective function, (2) these pathways are dysregulated in affective disorders, and (3) activating warm thermosensory pathways promotes a sense of well-being and has therapeutic potential in the treatment of affective disorders. Background This paper examines the degree of agreement among simple dichotomous self-report, validated screening results, and biochemical screening results of prenatal alcohol and other drug use among pregnant women. Method Secondary analysis was conducted on a cohort of pregnant women 16 years or older, presenting for prenatal care in the greater Cape Town, South Africa. Dichotomous verbal screening is a standard of care, and pregnant patients reporting alcohol and other drug use in dichotomous verbal screenings were asked to engage in (...) screening using the Alcohol Smoking and Substance Involvement Screening Test (ASSIST) and urinalysis. Results Significant agreements between dichotomous and ASSIST scores were observed (K = 0.73–0.76). A higher rate of self-reported (36.9%) alcohol use was detected, relative to urine screening (19.6%) with a predictive value of 34.9; while underreporting of illicit substance use was observed (3.6% self-report vs. 8.8% urine screening) with an overall predictive value of 50.0. Conclusion Dichotomous verbal screening was considered valid after comparison with the ASSIST; however, combined use with urine screenings can be recommended especially for identifying illicit substance use in order to accurately detect alcohol and other drug use in pregnancy, so that women can be identified and referred for appropriate interventions where needed.
